I think on top of saving at timed intervals, it also saves whenever anything on your character changes, such as picking up an item. So it probably saved after you picked up the hat, but then the timed interval save afterwards failed. So probably there wasn't anything to save anyways.

I dont think it saves when anything on your character changes, there have been many claims of disconnection after picking up an item causing loss of said item. i think it had something more to do with the "Quitting, Please Wait" in the chatbox.
